My Life a Full Life is a new way of working across our health and care system – our new care model - on the Isle of Wight, that will enable us to manage our current and future challenges ensuring safe, sustainable, quality care and health improvements for all. A system in which health and care services will work together in a more coordinated, effective and efficient way, delivering more care at home and in the local community, to enable people to manage their health more easily and live their lives to the full.

My Life a Full Life has been developed with the support and guidance of all the key voluntary, community and independent organisations that provide health and care on the Island (our Local Delivery System), the Clinical Commissioning Group (the group of NHS staff who choose and buy health services), the Isle of Wight NHS Trust (which provides ambulance, community, hospital, mental health and learning disability services), the Isle of Wight Council (which provides a range of services including public health support, social care, education and housing), One Wight Health (a GP group), Hampshire Constabulary and Town and Parish Councils. Our local community has also been involved through events, community discussions, surveys and projects to help shape our new care model.
A Local Care Board has been established bringing together all the key partners to unite efforts to improve the overall quality of health and social care on the Isle of Wight. The Board has developed a Local Care Plan for the Island prioritising six core initiatives that will enable the continued delivery of our new model of care, ensuring personalised care is available closer to home.
