Helping you navigate the care system

Care Navigators are a team of local experts who fully understand the health and social care support options available and help people ‘navigate’ the right routes for them.

Employed by Age UK Isle of Wight and originally part-funded through My Life a Full Life, they work from GP surgeries across the Island with the majority of their referrals coming from GPs, but also from health and social care professionals or self-referrals. Over 2016 the nine Care Navigators across the Island made a great difference to 1,900 (2.7%) of our aged 50+ population.

How they work:

  • They undertake an initial assessment looking overall at what a person needs for a ‘good’ life
  • They allow for up to 6 visits to access appropriate services to best meet that person’s needs and to promote and support independence and self-management
  • They try where possible to enable independence at home and as such 58% of the support they have provided has come from the voluntary sector
