We are working to ensure that children on the Isle of Wight are supported to get the best start in life that will lead to good health and wellbeing. This will provide the foundation to ensure they are able to achieve the best opportunities and keep as healthy and well as possible throughout their lives.
We want to ensure that families, individuals and communities are thriving and resilient with access to good jobs, affordable housing, leisure activities, lifelong training, education and learning, health and care services are are able to enjoy the place in which they live.
We want to ensure that people on the Isle of Wight are able to live independently in their own homes with appropriate care support. We want to make sure older residents are supported to play an active role in their communities and supported to maintain and develop their social and community networks.
Mr L was overweight, living with his elderly mother and had challenging financial issues.
He was in a downward spiral of financial pressures, having to care for his mother and depression affecting his weight.
What was put in place to help:
The Care Navigator identified the nub of the problem was to firstly help deal with the financial issues. They helped him to secure a claim for PIP as well as recoup long distance travel expenses to/from the hospital.
They approached the Housing Association to assist with selected repairs to the property, and also put them on a waiting list for Occupational Therapy support.
How things are working now:
The financial support enabled him to address the most pressing issues and has led to them both living in a safer home environment. It has also boosted his self esteem and he has since been working regularly with a Health Trainer to try and manage his weight and health.
