We are working to ensure that children on the Isle of Wight are supported to get the best start in life that will lead to good health and wellbeing. This will provide the foundation to ensure they are able to achieve the best opportunities and keep as healthy and well as possible throughout their lives.
We want to ensure that families, individuals and communities are thriving and resilient with access to good jobs, affordable housing, leisure activities, lifelong training, education and learning, health and care services are are able to enjoy the place in which they live.
We want to ensure that people on the Isle of Wight are able to live independently in their own homes with appropriate care support. We want to make sure older residents are supported to play an active role in their communities and supported to maintain and develop their social and community networks.
Mrs W has advanced Dementia and is living at home and being cared for by her family. They have not wanted any medical interventions but have sought support in signposting and information.
As Mrs W’s Dementia advanced, her needs also increased. Her family seemed to be coping well caring for her but there were times when they needed guidance as to where to find help around specific areas such as application for a Blue Badge, an continence nurse etc.
What was put in place to help:
It was essential the Care Navigator build a relationship of trust with the family to respect their wishes and support their needs. They focussed around providing help which would not interfere medically but improve the care they were providing themselves. They introduced them to an Attendance Allowance and supported them in their application; directed them to some useful websites on Dementia; and referred them to a continence service.
How things are working now:
The family has continued to care for Mrs W successfully themselves and has maintained an intermittent relationship with the Care Navigator, asking for guidance as and when they have needed it.
