Case study 4.

Following a stay in hospital, Mrs L returned home but very quickly realised her new state of health needed more support than she or her husband, who was suffering from Dementia, were able to give.

She was unable to manage her personal care herself, and her husband’s condition meant he struggled to process her instructions and help her. Time was of the essence in order to prevent potential falls and distress.

What was put in place to help:

Social Services were brought in urgently. They initiated an immediate daily care call and the provision of a home visit physiotherapist.

How things are working now:

Mrs L was able to regain her mobility, and in turn she and her husband were able to return the lives they had enjoyed prior to her hospital stay.
