Case study 8.

Middle aged, Miss X lives alone and suffers from impaired eyesight and several other difficult health issues. Collectively they challenged her confidence and she struggled to look after herself. As such she had become predominantly house bound and isolated.

What was put in place to help:

The Care Navigator arranged for life enhancing aids to be installed in her home such as grab rails in the shower. They also introduced her to some local social clubs and to some like minded people.

How things are working now:

She joined several clubs which she regularly attends and has met many new friends. The enhancements to her home have meant she is better able to look after her own needs. As a result of these interventions, her confidence has grown dramatically and, with it, her happiness.
